The purpose of this section of the website is to provide clergy with materials you may need in the carrying out of
your ministerial duties. Please feel free to click on any link and download the material and use as you see fit.
All files are written as Word documents.
If you do not see something here that you need or would like to have, please let Bishop Martel know and it will be
uploaded in the near future.
Once you see what you want, print it out. BUT, before you do so, make the appropriate page breaks and fill it the
mission information. I have made notations in red which you should read and delete before printing our document.
CCA Directory. This directory contains current clergy members as well as all persons who have at one time
or another written to us inquiring for information about the CCA. All names are being maintained for future
use.
Membership Form. This form should be used when a new person is admitted into CCA church
membership whether if done by Baptism, Chrismation, reaffirmation or transfer.
Groups at Church. Some suggestions on how to get people at your church building or parish center or
location.
Accounting. Clergy can check here on the status of their monthly donations and any other indebtedness
they have have.

Sign In Sheet. A sample Sign In Sheet to be placed in the back of the church or building you are using .
Vehicle Sign. Please laminate and post in windshield when on an emergency or when "on call."
Monthly Tithe Form. To be submitted each month to the office of the National Treasurer. Form is to be used
by both parishes and ministries.
Parish Survey. This is an excellent survey to give out to all members of your parish to get more
involvement in the parish. It is also a great tool to give to new church members as they express an interest
in becoming church members. This will show them how they can get involved and that their involvement is
wanted and needed.
